For our patients

At Center For Urology, patient care is our number one priority. We will work with you to schedule an appointment with the physician of your choice. If that physician isn't available we will suggest another qualified member of our group for you to consider. When you call to schedule an appointment, please feel free to confidently disclose the nature of your medical problem, so we can ensure enough time for your visit.

Preparing for your Appointment

Appointments can be made via telephone or in person. Please have your insurance information available for the secretary when you call. When you come to the office, please bring the following information:

  • A list of medications that you are currently taking
  • Your insurance card(s)

Center For Urology participates with most major insurance companies. Please contact your insurance provider to verify our participation. While the fee is the patient's responsibility at the time of treatment, we will gladly assist you in filing insurance claims. Be sure to present your insurance information to our office staff at the time of your visit. Please be familiar with your insurance plan so that if referral paperwork is required, you will have it available before, or at the time of, your visit.
